There is no cure for Type 2 diabetes. Treatment, then, aims to help people manage lifestyle changes in order to control their blood glucose levels. This minimizes the risk of complications in people with the condition, or can prevent Type 2 diabetes developing in the first place.
Exercise and a healthy diet are recommended both for prevention and for people already diagnosed. People with diabetes often have other medical problems or risk factors for ill health, such as heart disease, high blood pressure, a history of stroke or obesity. In these cases, it's especially important that your lifestyle changes, including your exercise program, meet your individual needs.
Your physiotherapist is the key health professional to advise about this. In trying to prevent the onset of diabetes, or to stop the condition becoming worse, a physiotherapist will advise on exercise to reduce body fat and to improve how the body processes glucose. Both are key factors in managing diabetes.
Physiotherapists also have a role in treating people with some of the complications of diabetes. These include difficulties with vision and sensory changes due to circulation problems and nerve damage, both of which affect a person’s safety. Some people may develop a dropped foot, causing them to trip, which requires exercise and also assessment for a foot splint. In extreme cases, the altered circulation may result in an amputation. Here, a physiotherapist will be part of the team that gets you walking again.
Physiotherapy Management of Diabetes
Therapeutic exercise programs comprise the major aspect of management. Patient education for proper foot care is an essential part of the physical therapy program for diabetic patients.
Exercise Therapy
A sound, individually tailored exercise prescription is a cornerstone in the management of Diabetes Mellitus.
It is recommended that people with DM ought to have a regular aerobic exercise and strength training to reassure positive adaptations in the control of blood glucose concentration, insulin action, muscular strength and exercise tolerance.
Importance of different modes of exercises in patients with type 2 diabetes - increases uptake of glucose by muscles, improves utilization, altering lipid levels, increases high density lipoprotein and decreases triglyceride and total cholesterol.
Exercise helps people to overcome disability by preventing, treating and rehabilitating neuromuscular complications like neuropathies, skin break down, foot ulcers, arthritis, other joint pains, frozen shoulder, back pain and osteoarthritis associated with DM.
Moderate to high levels of different modes of exercises like cardio respiratory fitness exercises, aerobic exercise and progressive resistance exercises are also associated with substantially lower morbidity and mortality in men and women with diabetes.
Guidelines for a sound exercise program are as follows:
Do not exercise if the blood glucose level is less than 100 mg/dl or greater than 250 mg/dl.
Preferably, exercise indoor instead of outdoor to minimize the risk of integumentary and musculoskeletal trauma, as well as for the patient to have an immediate access to necessary things to address hypoglycemia, hyperglycemia or diabetic ketoacidosis.
Patients are highly advised to wear the medical tag for diabetics each time they come out of their house to go somewhere else.
Always have a carbohydrate snack at hand every exercise session. A glass of orange juice or milk is a good pickup for a patient who is experiencing hypoglycemia.
Exercise in a comfortable temperature. Never exercise in extreme temperatures.
For Type 1 (Insulin Dependent) patients, never exercise during the peak times of insulin. Collaborate with the nurse in charge for the patient regarding the type of insulin administered.
Type 2 diabetics are advised to have an average of 30 minutes of exercise duration per session.
Always wear proper footwear and exercise in a safe environment.
Type 1 diabetics may need to reduce insulin or increase food intake prior to the start of an exercise program. Physical Therapists must coordinate with the referring physician for this case.
During prolonged exercise duration, 10-15 grams of carbohydrate snack is recommended for every 30 minutes.
Clients who are on Sulfonylureas are red flags because it can cause exercise-induced hypoglycemia. Closely coordinate with the referring physician if this was missed prior to referral.
Menstruating women need to increase insulin during menses, especially if they're not active.
There should be no short-acting insulin injections close to the muscles to be exercised within one hour of exercise.
Patients should eat 2 hours before exercising. If planning to exercise after meal, patients must wait 1 hour prior to start.
Patients must always carry their own portable blood glucose monitor. They must check their glucose levels before and after exercise.
Patients are advised to drink 17 oz. of fluid before exercise.
If blood glucose is less than 100 mg/dl but not less than 70mg/dl, the physical therapist may provide carbohydrate snack and then recheck the glucose level after 15 minutes.
Make sure exercise doesn't contribute unnecessary stress to the patient. Stress increases insulin requirements. A gradual progression from aerobic and resistance exercises is the key.
Avoid exercising late at night.
If faced with an unexpected and difficult situation wherein the physical therapist is in doubt whether the patient is experiencing hyperglycemia or hypoglycemia, always give a glass of orange juice or milk, or a carbohydrate snack. This is the safest action because this can relieve hypoglycemia (if it is indeed) and cannot harm if it is hyperglycemia.
Exercise five times a week as a maintenance (or at least every other day) and at the same schedule / time, preferably.
As much as possible, patient must not exercise alone, so that there will always be someone to help in unexpected situations.
Good examples of carbohydrate snacks (10-15 grams of carbohydrates) are half a cup of fruit juice or cola, 8 oz. of milk, 2 packets of sugar, 2 oz. tube of honey or cake deco gel.
Diabetics are more prone to hypoglycemia than hyperglycemia during exercise. Watch for classic Signs and Symptoms
Lifestyle Changes to Control Your Diabetes
Eat healthy. This is crucial when you have diabetes, because what you eat affects your blood sugar. No foods are strictly off-limits. Focus on eating only as much as your body needs. Get plenty of vegetables, fruits, and whole grains. Choose nonfat dairy and lean meats. Limit foods that are high in sugar and fat. Remember that carbohydrates turn into sugar, so watch your carb intake. Try to keep it about the same from meal to meal. This is even more important if you take insulin or drugs to control your blood sugars.
2. Exercise. If you're not active now, it’s time to start. You don't have to join a gym and do cross-training. Just walk, ride a bike, or play active video games. Your goal should be 30 minutes of activity that makes you sweat and breathe a little harder most days of the week. An active lifestyle helps you control your diabetes by bringing down your blood sugar. It also lowers your chances of getting heart disease. Plus, it can help you lose extra pounds and ease stress.
3.Proper Care of limbs.
Ensuring your limbs remain clean and moisturized is key to preventing and treating diabetic neuropathy. Make sure to wash your hands and feet properly when showering or bathing. Use anti-bacterial soap or body wash, but make sure to rinse away the soapy residue with warm water.
After bathing, spend extra time drying off your hands, feet, legs, and arms with a clean, soft towel. Once you are dry, moisturize your hands and feet using a specialized lotion. Coconut oil is also a great option to use on your hands and feet. A few tablespoons of coconut oil is sufficient for your hands and feet. Not only does it feel good during application, but its anti-bacterial and anti-fungal properties protect your feet from possible infections.
4.Get checkups
See your doctor at least twice a year. Diabetes raises your odds of heart disease. So learn your numbers: cholesterol, blood pressure, and A1c (average blood sugar over 3 months). Get a full eye exam every year. Visit a foot doctor to check for problems like foot ulcers and nerve damage.
5.Manage stress
When you're stressed, your blood sugar levels go up. And when you're anxious, you may not manage your diabetes well. You may forget to exercise, eat right, or take your medicines. Find ways to relieve stress -- through deep breathing, yoga, or hobbies that relax you.
6. Stop smoking
Diabetes makes you more likely to have health problems like heart disease, eye disease, stroke, kidney disease, blood vessel disease, nerve damage, and foot problems. If you smoke, your chance of getting these problems is even higher. Smoking also can make it harder to exercise. Talk with your doctor about ways to quit.
7. Watch your alcohol
It may be easier to control your blood sugar if you don’t get too much beer, wine, and liquor. So if you choose to drink, don't overdo it. The American Diabetes Association says that women who drink alcohol should have no more than one drink a day and men should have no more than two. Alcohol can make your blood sugar go too high or too low. Check your blood sugar before you drink, and take steps to avoid low blood sugars. If you use insulin or take drugs for your diabetes, eat when you're drinking. Some drinks -- like wine coolers -- may be higher in carbs, so take this into account when you count carbs.
Pain management for diabetic neuropathy through Massage Therapy:
High blood sugar can result in the thickening of connective tissue, and massage will increase the mobility and elasticity of that tissue. As the muscles, tendons and ligaments are relaxed, patients will find an increase in motion and flexibility in the joints, therefore improving their quality of life.
What is Diabetic Neuropathy?
If you have diabetes, you most likely have high levels of blood sugar. This excessive amount of blood sugar can damage the nerve fibers all through your body. When this damage affects the nerves in your hands, feet, arms, and legs, neuropathy occurs.
If you are diabetic and experience one or more of the following in your hands, feet, arms, and legs, you may have neuropathy:
*Tingling
*Numbness
*Burning
*Sensitivity to touch
*Weak muscles
*Balancing/coordination issues
*Sharp pain
*Infections
*Joint/bone deformities
Massage Benefits
Professional massage can help you with your diabetic neuropathy. Massage improves the circulation of blood through your body, which reduces inflammation and pain while promoting healing. During massage, your brain will release endorphins. These neurotransmitters flow through your body, instantly improving your mood and managing your stress.
If you suffer with diabetes, consider visiting a massage therapist to stimulate nerve endings in your limbs. This will reduce your risk of developing neuropathy. However, if you are currently experiencing the discomfort associated with neuropathy, schedule regular sessions with your massage therapist to reduce your pain and improve your stress level. If you are not able to schedule an appointment with a therapist then you yourself or a caregiver can give massage to the feet every night, before going to bed which will help in avoiding foot neuropathy.
